About the role

The LPN plays a crucial role in providing direct nursing care to residents while supervising nursing assistants. This position requires adherence to federal, state, and local standards and guidelines.

Responsibilities

  • Direct the day-to-day functions of nursing assistants.
  • Plan shift services, programs, and activities with assigned nursing staff and support personnel.
  • Make written and oral reports as required.
  • Admit, transfer, and discharge residents as needed.
  • Complete and file required record-keeping forms and charts.
  • Transcribe and record physician’s orders on the Physician’s Order Form.
  • Chart nurse’s notes in a detailed manner reflecting care provided and resident responses.
  • Fill out and submit accident/incident reports to the Director as necessary.
  • Perform routine charting duties in accordance with established policies and procedures.
  • Administer medications as ordered by the physician.
  • Create and maintain a positive and supportive environment for residents.
  • Receive and give the nursing report upon shift change.
  • Administer professional services such as catheterization, tube feedings, suction, dressing changes, and massages.
  • Maintain confidentiality of resident care information.
  • Ensure all nursing care is provided in privacy and that personnel knock before entering the resident’s room.
  • Perform other duties as assigned.

Qualifications

  • Current licensed practical nurse with an unencumbered state license.
  • Ability to read, write, speak, and understand English.
  • Knowledge of nursing and medical practices, as well as relevant laws, regulations, and guidelines.
  • CPR certification required.
  • Experience in long-term care is preferred.

Physical Demands

  • Must be able to move intermittently throughout the workday.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must function independently, have flexibility, personal integrity, and effective communication skills.
  • Must be able to lift and move up to 25 pounds and carry items up to 50 feet.
