About the role

The LPN is a vital member of the interdisciplinary team providing quality patient-centered, evidenced-based care to our residents. Responsibilities include initial data collection, implementation of person-driven care plans, and adherence to the principles of Relationship Based Care.

Responsibilities

• Initial data collection on all residents
• Participate in the development and implementation of person-driven care plans
• Utilize principles of Relationship Based Care

Requirements

• Current state licensure as a Practical Nurse, in good standing
• Ability to read, write, and communicate in English
• Clinical experience preferred

Qualifications

• Highly skilled in providing compassionate and quality care
• Strong interpersonal and communication skills
• Willingness to work in a fast-paced, supportive environment

• $26.71 - $28.46 per hour
