About the role

ReMed, a Collage Rehabilitation Partner, is seeking a full‑time Licensed Practical Nurse (LPN) to work in its 8‑bed residential program in West Chester, PA. The LPN works under the direction of a Registered Nurse, providing nursing and facilitation services for clients with moderate to severe traumatic and acquired brain injuries, including post‑concussion syndrome, stroke, memory loss, neurobehavioral issues, and accident or sports‑related head injuries.

Responsibilities

• Utilize nursing knowledge and skills to safely implement basic preventative, therapeutic, and rehabilitative nursing care for assigned clients, documenting outcomes.
• Identify individuals in need of medical nursing services or interventions in a timely manner and provide appropriate interventions.
• Perform hands‑on nursing and facilitator care using the nursing process, applying specialized clinical knowledge.
• Train and educate families, funders, employers, and other constituents, developing individualized teaching and discharge plans.
• Provide direct care such as grooming, bathing, and feeding to clients at program locations or during medical appointments as needed.

Requirements

• Current state licensure as a Licensed Practical Nurse (or equivalent).
• Experience in rehabilitation of chronically or catastrophically ill or injured individuals (preferred).
• Demonstrated accountability and skills in analysis, problem solving, decision making, and time management related to medical nursing.
• Strong written and verbal communication skills for interaction with families, clients, staff, and physicians.
• Valid driver’s license and acceptable driving record maintained at all times.
• Competence in writing, planning, organization, documentation, and time management.
