About the role

Join us in Simplifying Complex Care! The position involves providing primary patient care, evaluating and educating patients, developing care maps, coordinating care, and documenting services. This role requires a license as an LPN in Arizona or a CMA certification, with at least 3 years of infusion therapy experience and IV certification.

Responsibilities

  • Provide direct care for assigned patients, including initiating and maintaining communications with care team members.
  • Evaluate and educate patients on administering home infusion therapy and provide additional instructions.
  • Evaluate home environments for safety and appropriate care.
  • Create and update patient care schedules and communicate with the team.
  • Document all communications and vital signs.
  • Assist with vital sign monitoring, PICC line dressing changes, PIV insertions, and administrative tasks.
  • Schedule home visits and coordinate care through phone conversations.

Requirements

  • Licensed as an LPN in Arizona or certified as a CMA via a national exam.
  • Graduate from an accredited LPN or LVN program.
  • Current IV certification in compliance with state and organizational standards.
  • At least 3 years of nursing experience with a focus on infusion therapy.
  • Proficient in Microsoft Excel, Word, and Outlook.
  • Excellent customer service skills.

Qualifications

  • Knowledge of care matrices and best practices in home care settings.
  • Ability to develop and implement care plans.
  • Experience with EMR documentation.
  • Strong communication and teamwork skills.
