Licensed Practical Nurse (LPN)/Medical Assistant (MA)

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Assists patients with completion of history and physical forms.
• Affixes vital signs as needed, including height, weight, blood pressure, pulse, respirations, and temperature.
• Rechecks vital signs as needed, including height, weight, and blood pressure.
• Checks pulse oximetry and blood sugar as needed.
• Documents and confirms patient information in each visit.
• Assesses physical, psychological, and educational needs of patients.
• Monitors and documents patient care needs and changes in condition.
• Collaborates in coordinating the patient's plan of care.
• Prepares and transfers patients within the facility or to other facilities.
• Administers injections under supervision.
• Ensures accurate and complete documentation in the EMR or paper chart.
• Provides patient and family/significant other teaching.
• Documents patient and family/significant others' educational needs.
• Documents patient responses to interventions.
• Communicates with providers promptly and accurately.
• Follows through on provider messages and urgent orders.
• Coordinates with insurance companies for pre-certifications and pre-authorizations.
• Communicates with other healthcare professionals to obtain and follow through on provider orders.
• Reports on patient conditions to relevant parties.
• Participates in the general operation and organization of the Nursing unit.
• Supports the health care team and maintains a positive attitude.

EDUCATION/QUALIFICATIONS

• Graduate of an accredited school of practical nursing.
• Current licensure in the State of Illinois for practice as a licensed practical nurse.
• Current CPR certification or willingness to become certified.
• Completion of EPIC training and successful passing of required exams.
