Licensed Practical Nurse (LPN/LVN) with Compact/Multistate License

Responsibilities

• Serve as the primary contact for patients on the RPM365 platform
• Affiliate with device setup and app connectivity
• Engage with patients to ensure adherence to care plans
• Schedule monthly check-ins and monitor patient data
• Educate patients through shared resources
• Notify healthcare providers of critical alerts
• Attend virtual team meetings for updates

Qualifications

• Minimum: Active nursing license that is a compact/multistate
• Knowledge of chronic conditions and related monitoring devices (weight management a plus)
• Proficiency with technology, computer platforms, and smartphones
• Strong verbal/written communication skills
• Comfortable with patient outreach and engagement
• Highly organized, detail-oriented, and team-focused
• Independently motivated

Preferred

• Experience in telehealth, patient monitoring, education, or customer service
• Additional language skills

Compensation & Benefits

• Salary: $45,000-$49,000 (DOE) + performance bonuses
• Schedule: Mon-Fri, 9:00 AM - 5:00 PM EST (Remote)
