LICENSED PRACTICAL NURSE (LPN) - LONG TERM CARE | FULL-TIME FLOAT POOL EVENING SHIFT (22808)

Responsibilities

• Patient Care: Collects data from patient, family or significant others and records on Initial Data Base form. Twenty-four hour nursing assessment and provides data to RN for MDS inclusion.
• Formulates and updates plan of care and implements plan of care under the direction of the Registered Nurse. Gives direct nursing care to assigned patients (e.g., medications, treatments, dressing changes, resident care, TPR) according to established standards.
• Recognizes changes in patient condition and responds appropriately, notifying charge nurse as needed. Documents pertinent information regarding patient's responses and nursing interventions in patient's medical record.
• Administers medication according to hospital policies; understands and monitors age specific data including meds, lab/diagnostic results, etc.
• Applies principles of infection control and safety in the performance of patient care. Communicates effectively with charge nurse and other members of the health care team regarding patient's needs and/or changes in condition. Responds appropriately in crisis situation; able to prioritize. Reports significant unit problems to the charge nurse.
• Customer Service: respects, is flexible, knowledgeable, confident, professional, pleasant, patient, and helpful. All responses should be timely, professional, caring, and respectful in accordance with Customer Service Performance expectations.
• Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental, and infection control standards. Assesses gaps in policies and procedures and creates necessary policies and procedures to fulfill these gaps.
• Understands and adheres to the Medical Center’s Code of Conduct. Familiar with the Medical Center’s Mission, Vision, and Values Statements.
• Orients patient and family to new environment; develops rapport with patients, family, and/or significant others. Orients LPNs, Nurses’ Aides, and orderlies as required.
• Utilizes the hospital computer system as it pertains to job responsibilities and in accordance with hospital policy. Participates on unit committees and in CQI activities as assigned. Participates in patient care conferences and in staff development activities as required. Assists other personnel as needed.

Requirements

• Current NJ license as an LPN.
• Successful completion of Progressive Care Orientation Program required.
• American Heart Association Basic Life Support certification required.
• IV certification preferred.
• Clinical nursing skills appropriate for an LPN.
• Good oral and written communication skills.
• Good interpersonal skills.
• Speaks, reads and writes English to the extent required by the position.

Qualifications

• Graduation from an accredited school of practical nursing or equivalent education as described by the state education law is required.
