Licensed Practical Nurse (LPN) *Increased Incentive*

About the Role

The Licensed Practical Nurse (LPN) provides direct nursing care to residents and supervises the day-to-day nursing activities performed by nursing assistants. Supervision must align with current federal, state, and local standards, guidelines, and regulations governing the facility, ensuring the highest degree of quality care is maintained.

Responsibilities

• Direct the day-to-day functions of nursing assistants.
• Plan and coordinate shift services, programs, and activities with assigned nursing staff and support personnel.
• Provide written and oral reports concerning shift activities as required.
• Admit, transfer, and discharge residents as needed.
• Lead nursing personnel assigned to your unit or shift.
• Complete and file required record-keeping forms and charts for resident admission, transfer, and discharge.
• Receive and transcribe physician telephone orders onto the Physician’s Order Form.
• Chart nurse’s notes descriptively to reflect care provided and resident responses.
• Complete and submit accident/incident reports to the Director as required.
• Perform routine charting duties in accordance with established policies and procedures.
• Prepare and administer medications as ordered by the physician.
• Maintain good working relationships with inter-departmental personnel to ensure seamless nursing services.
• Foster a warm, positive, and calm environment for residents and staff.
• Conduct shift change reports and make physician rounds as necessary.
• Administer professional services such as catheterization, tube feedings, suction, wound care, colostomy care, blood draws, massages, range of motion exercises, and end-of-life care.
• Monitor assigned personnel to ensure compliance with safety regulations for equipment and supplies.
• Maintain resident confidentiality, including protected health information, and report unauthorized disclosures.
• Ensure resident privacy and that nursing personnel knock before entering rooms.
• Perform other duties as assigned.

Requirements

• Licensed Practical Nurse with a current unencumbered state license.
• Ability to read, write, speak, and understand English.
• Capability to make independent decisions when circumstances require.
• Knowledge of nursing and medical practices, procedures, and relevant laws, regulations, and guidelines for nursing care facilities.
• Current CPR Certification.
• Experience in Long Term Care is preferred.

• Ability to move intermittently throughout the workday.
• Capacity to cope with mental and emotional stress associated with the position.
• Ability to function independently, demonstrate flexibility, personal integrity, and work effectively with residents, personnel, and support agencies.
• Capability to push, pull, move, and/or lift a minimum of 25 pounds to a height of 5 feet and transport such weight a minimum distance of 50 feet.
