Licensed Practical Nurse - LPN Hospice Visits

About the role

BAYADA Hospice is looking for compassionate and dedicated Licensed Practical Nurses (LPNs) to join the North Jersey Hospice office in North Bergen, NJ for per diem visits. This office services hospice clients on a per visit basis in North Jersey, primarily Essex, Union, and Middlesex counties. LPNs must be comfortable with hospice admissions.

Schedule

• Minimum of 2 weekdays availability required
• 1–2 holidays per year mandatory
• No on-call required
• Must be willing to cover any county – assignments will vary
• This is a visiting position – reliable transportation needed

Pay

Weekly pay. Base pay, depending upon qualifications and subject to negotiation: $38–$41 per hour.

Responsibilities

• Making home visits to clients in designated geographic territories
• Performing assigned duties, including administration of medication, wound care, treatments, procedures, and comfort care
• Monitoring clients' conditions; reporting changes to Clinical or Client Services Manager
• Following up with, executing, and properly documenting doctors' orders
• Performing client assessments as necessary
• Occasional on-call after hours and/or weekends

Qualifications

• A current license as a Licensed Practical Nurse in New Jersey
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience
• Prior home care and/or hospice experience strongly preferred, but not required
• Solid computer skills; prior experience with electronic medical records (EMR) preferred
• Ability to travel to cases as assigned
