About the Role
As a Home Health Licensed Practical/Vocational Nurse (LPN/LVN), you provide direct patient care under the supervision of a Registered Nurse, helping to implement care plans and ensure patients receive safe, effective treatment in their homes. You are a key part of the care team—delivering treatments, monitoring patient conditions, and reinforcing education to support successful outcomes.

Responsibilities
- Provide direct patient care in accordance with the state Nurse Practice Act.
- Implement and support the plan of care developed by the RN.
- Deliver skilled nursing services, including treatments, medication support, and wound care.
- Monitor patient condition and report changes to the RN and care team.
- Educate patients and caregivers on self-care, treatments, and disease management.
- Maintain infection control and proper use of equipment and supplies.
- Document patient care accurately and timely within the EMR.
- Communicate effectively with physicians, nurses, and interdisciplinary team members.
- Participate in case conferences, team meetings, and ongoing training.
- Support coordinated care by maintaining consistent communication across the care team.
Requirements
- Active LPN/LVN license in the state of practice.
- Graduate of an accredited practical/vocational nursing program.
- Current CPR certification.
- Minimum one (1) year nursing experience preferred (Home Health, Med-Surg, or Acute Care a plus).
- Strong clinical and observational skills.
- Clear communication and patient education abilities.
- Ability to follow care plans while recognizing and reporting changes in condition.
- Basic proficiency with EMR systems.
- Valid driver’s license and reliable transportation.
