In this position
- Provide nursing care, emotional support, patient and family education regarding disease process, self-care techniques, prevention strategies, clinical nursing observations, and documentation for Home Health patients.
- Provides nursing services as ordered by the physician and under the direction of the RN.
- Reports subjective and objective patient findings to the RN.
- Under the direction of the RN, obtains physician’s orders and ensures orders exist for each visit made.
- Provides necessary skills to report changes appropriately; documents pertinent information and care rendered to the patient to ensure continuity of care.
- Demonstrates knowledge of infection control standards and techniques as per VNHS policy and procedures and consistently practices these techniques.
Do you have...
- LPN Graduate from an accredited school of nursing or program
- Completion of an IV training program or certification in IV therapy
- Experienced in advanced wound therapies
- Current license by the Georgia State Board of Nursing.
- Minimum of one - two (1-2) years’ experience in a Medicare-certified home health agency is preferred.
- Minimum of two (2) years as an LPN
- Current valid Driver’s License, proof of auto insurance, and a reliable vehicle
Pay Range
$61,634.56-$92,451.84
