Responsibilities

• Deliver skilled nursing care in patients' homes according to physician orders and individualized care plans
• Perform comprehensive patient assessments and monitor changes in condition
• Educate patients and families to promote confidence and independence at home
• Collaborate with RNs, Physical Therapists, Occupational Therapists, Speech Therapists, physicians, and the interdisciplinary care team
• Maintain accurate documentation using our AI-supported charting technology
• Participate in quality improvement initiatives and ongoing professional development

Requirements

• Current Missouri or Compact LPN license
• Excellent assessment and communication skills
• Strong organizational and time management abilities
• Compassionate, patient-centered approach to care
• Valid driver's license and reliable transportation

Home health experience is a plus, but we welcome nurses who are eager to learn.

Schedule

Monday through Friday, 8:00 AM to 5:00 PM, with a weekend rotation approximately every 4 to 6 weeks.

Pay

$60,000 - $75,000 plus $2,500 sign-on bonus and paid mileage.
