About the Role
AdventHealth is seeking a Licensed Practical Nurse (LPN) to join our home health care team. This position involves providing direct patient care under the direction of an RN, performing treatments, administering medications, and maintaining clinical records.
Responsibilities
- Provides patient education, medication administration, wound dressing changes, and ostomy care as directed by the RN and ordered by the physician
- Provides direct patient care responsibilities for assigned patients in an efficient, organized manner under the direction of the RN
- Performs treatments within their scope of training and administers medications to assigned patients
- Assesses patients during clinical check-in, documents pertinent medical information and vital signs, and assesses for signs of abuse or neglect using age-specific guidelines
- Administers treatments, medications, and follow-up assessments ordered by the provider, verifying as needed, and responds appropriately in clinical emergencies and triage situations
- Performs diagnostic and occupational medicine testing, including EKG, spirometry, audiometry, titmus, and pulse oximetry, and complete forensic testing collections in accordance with established guidelines
- Collects data specific to the age of the patient and contributes to their plan of care
- Receives, verifies, and implements physician orders, ensuring orders taken by the LPN are co-signed by an RN
- Reports pertinent observations to the attending physician and RN, and implements changes in the patient care plan as directed
- Communicates changes to the appropriate members of the home health care team
- Demonstrates effective communication skills with all team members, patients, and families
- Maintains clinical record information in accordance with established standards and within established time frames
- Participates in the review process and the agency performance improvement program
- Develops professional and clinical skills through attendance at staff development meetings, pertinent educational activities, and professional affiliations
- Displays flexibility in accepting assignment changes and participates in weekend/after-hours coverage and call rotation

Qualifications
- High School Grad or Equivalent [Required]
- Technical/Vocational School [Required]
- Field of Study: LPN
Required Licenses & Certifications
- Licensed Practical Nurse (LPN) [Required]
- Basic Life Support - CPR Cert (BLS) [Required]
- Driver's License (DL) [Required]
- Auto Insurance (AL) [Required]
- IV Certified (IV) [Preferred]

- Must have excellent communication skills, be flexible, and able to interact positively with associates [Required]
- Additional computer knowledge [Preferred]
- Knowledge with a Point of Care software system for patient care documentation [Preferred]
- Additional languages [Preferred]
Experience
- 1+ year of medical/surgical experience [Preferred]
Schedule
Full Time: Monday – Friday, 8am – 5pm, flexible, rotating weekends on-call

$20.97 - $38.99
