Salary: $48,000-$59,000
Schedule: M-F 8a-4:30p
On Call: LPN on call one weekend a month (Sat, Sun); no weekday call
This position is paid on a per-point basis.

Responsibilities

• Performs technical nursing functions and direct patient care consistent with the established plan of care and under the supervision of an RN.
• Promotes and maintains optimal client care under the supervision of an RN.
• Maintains a safe patient environment.
• Identifies and communicates significant abnormalities in patients data to case manager and other appropriate home health team members.
• Administers prescribed medications and treatments as permitted by law.
• Affords and evaluates all medicines a client may be taking to identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, contraindicated medications and reports any problems to the physician.
• Re-evaluates client’s nursing needs and reports findings to Case Manager.
• Counsels and instructs the client and family in meeting nursing and related needs.
• Prepares clinical and progress notes.
• Submits all documentation in a timely manner in accordance with agency policy.
• Documentation on patients chart correlates with nursing care plan.
• Communicates frequently and works closely with other home care team members.
• Aids in coordinating services provided to client.
