Licensed Practical Nurse, LPN - Home Care

About the role

The Licensed Practical Nurse (LPN) is responsible for providing nursing care to residents/patients, including medications and treatment administration, documentation, and other therapeutic interventions under the direction of the Clinical Director.

Essential Job Responsibilities

• Provides nursing care to patients within the scope of practice of a Licensed Practical Nurse.
• Coordinates and directs patient care.
• Collaborates with Registered Nurses for assessments, documentation, and data collection.
• Administers medications and completes treatments as ordered.
• Monitors patient's condition and reports changes as appropriate.
• Safeguards patients and team members, following infection control policies.
• Maintains a clean, safe, and organized work environment.
• Performs other duties as assigned.

Driving Requirements

• Local travel required
• Driver’s License required

Minimum Required Qualifications

• Accredited school of nursing degree for the Licensed Practical Nurse program
• Current LPN licensure in the state of employment
• Valid Drivers License, proof of insurance, and satisfactory motor vehicle record
• Effective communication skills in both written and verbal formats
• Basic computer skills
• Compliance with safety rules, regulations, policies, and procedures

Preferred Qualifications

• Previous experience in senior care or healthcare settings
