Licensed Practical Nurse (LPN) – Hall 1 Sub-Acute Rehabilitation & Long-Term Care

About the role

Hall 1 is a 40-45 bed Sub-Acute Rehabilitation and Long-Term Care unit caring for residents with high-acuity, medically complex needs. Our team provides comprehensive nursing care while helping residents achieve the highest possible level of independence and quality of life.

Responsibilities

• Deliver individualized nursing care based on resident assessments and care plans
• Administer medications and treatments in accordance with physician orders
• Monitor resident conditions and communicate changes to the interdisciplinary team
• Perform focused nursing assessments and document care accurately
• Collaborate with physicians, therapists, and other healthcare professionals
• Support residents and families through education and compassionate communication
• Assist with daily unit operations and serve in a leadership role when needed
• Promote a positive, team-focused culture while ensuring excellent resident outcomes

Qualifications

• Education: Graduate of an accredited School of Practical Nursing
• Licensure & Certifications: Current Maryland Licensed Practical Nurse (LPN) license, American Heart Association (AHA) CPR Certification
• Experience: Previous long-term care, rehabilitation, or sub-acute experience is a plus

Skills

• Basic computer proficiency
• Experience with or willingness to learn: Electronic Medical Records, IV Pumps, Patient monitoring systems, Glucose monitoring (Accu-Chek), EKG equipment, Specialty bed operation
