Responsibilities
- Administer medications
- Provide wound care and skilled nursing procedures
- Assist residents with activities of daily living
- Supervise nursing assistants
- Monitor residents' physical and emotional status
- Develop individualized care plans in collaboration with the interdisciplinary team
- Accurate clinical documentation
- Coordinate admissions and discharges
- Participate in quality assurance
- Staff education
- Regulatory surveys
Qualifications
- Active LPN/LVN license in the state of practice
- CPR/BLS certification preferred
- Minimum of one year of clinical nursing experience; long-term care experience preferred
- Strong clinical observation, medication, and documentation skills
- Knowledge of long-term care regulations
- Effective communication, independent work ability, and clinical judgment
- Experience with electronic health records and Microsoft Office
