About the role
We are seeking a compassionate Licensed Practical Nurse (LPN) to provide direct patient care in a long-term care setting.
Responsibilities
- Administering medications, wound care, and skilled nursing procedures
- Assisting residents with activities of daily living to ensure comfort, dignity, and safety
- Supervising nursing assistants and medication aides during shifts
- Monitoring residents’ physical and emotional health, reporting changes to physicians
- Developing, implementing, and reviewing individualized care plans
- Completing accurate clinical documentation
- Managing resident admissions, transfers, and discharges
- Participating in quality assurance, staff education, and regulatory surveys
- Performing additional duties as assigned
Requirements
- Active LPN/LVN license in the state of practice
- CPR/BLS certification preferred
- Minimum one year of clinical nursing experience; long-term care preferred
- Experience in supervisory roles is a plus
- Strong clinical observation, medication administration, and documentation skills
- Knowledge of long-term care regulations
- Effective communication skills and ability to work independently
- Familiarity with EHR systems and Microsoft Office
