Responsibilities

• Supervised and delegated by a registered nurse, provides direct patient care in accordance with applicable scope and standards of practice.
• Reports to the Director or Manager of Department.
• Contributes to the assessment of patients by collecting data for analysis by an RN.
• Performs plan of care interventions, including specimen collection and wound care; may administer medication.
• Documents patient care provided.
• Aids in the development of the plan of care.
• Aids patients with daily living activities including personal hygiene, elimination, nutrition, and ambulation.
• Maintains regular and reliable attendance.
• Assists RN with communication of patient status and responses to interventions with RN, physician, patient, and family.
• Advocates for the physical and emotional well-being of the patient.
• Participates in performance improvement and continuing education.
• Provides quality patient care and works flexible shifts/units as needed.
• Performs other duties as assigned.

• Patient Populations Served: Neonate, Infant, Early Childhood, Late Childhood, Adolescent, Young Adult, Middle Adult, Older Adult.
• Protected Health Information Accessed: Demographic, Clinical, Insurance, Financial, Complete Medical Record.
• Physical Requirements: Frequent standing, walking, lifting/moving patients, and repetitive hand/arm movements.
• Exposure: Occupational exposure to bloodborne pathogens is present for all employees.
• Physical Hazards: Regular exposure to biological and chemical hazards, including blood and bodily fluids, hazardous medications, and latex.

What Are We Looking For

• Education/License: Graduate of accredited school of nursing, Current LPN license in state of practice, BCLS required.
• Other Certifications: Handle with Care preferred.
