Licensed Practical Nurse ( LPN ) – Evening Shift

Moundville Health & Rehabilitation is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our skilled nursing and rehabilitation team on the 3:00 PM–11:00 PM evening shift. As an Evening Shift Licensed Practical Nurse (LPN), you'll provide exceptional nursing care to residents in our long-term care and rehabilitation community while working alongside Registered Nurses (RNs), therapists, physicians, and other healthcare professionals dedicated to delivering outstanding resident-centered care.

About the role

Full-time position on the evening shift (3:00 PM–11:00 PM) in Moundville, AL.

Responsibilities

• Provide direct nursing care to residents in a skilled nursing, rehabilitation, and long-term care setting.
• Administer medications, treatments, and nursing interventions according to physician orders.
• Assess, monitor, and document changes in resident conditions.
• Monitor resident response to medications and treatments and communicate concerns to the interdisciplinary team.
• Assist residents with activities of daily living (ADLs) as needed.
• Collaborate with rehabilitation staff to support residents' recovery goals and individualized care plans.
• Maintain accurate electronic nursing documentation in accordance with state and federal regulations.
• Promote a safe, clean environment while following infection prevention and safety protocols.
• Supervise and support Certified Nursing Assistants (CNAs) to ensure quality resident care.

Requirements

• Current Licensed Practical Nurse (LPN) license in good standing.
• Current CPR/BLS certification.
• Experience in skilled nursing, rehabilitation, long-term care, post-acute care, or geriatric nursing is preferred but not required.
• Strong clinical assessment, medication administration, and communication skills.
• Ability to work collaboratively within an interdisciplinary healthcare team.

Pay

Base pay: $25.00–$29.55 per hour (based on experience). $10,000 sign-on bonus.
