Licensed Practical Nurse LPN: English-Speaking with Additional Language Proficiency

Per diem role for Western Connecticut (Fairfield, New Haven, Litchfield County) seeking bilingual Licensed Practical Nurses (LPNs) whose first language is English and who also speak an additional language.

Responsibilities

  • Assess needs of clients to develop a plan of care.
  • Consult with management on complex/unique cases.
  • Provide a plan of care with support of clinical/medical/multidisciplinary members to ensure services follow the plan of care through assessment, planning, coordination, counseling, teaching, and supervision.
  • Monitor and assess patients'/clients' condition and effects of the care plan.
  • Oversee and supervise care provided to patients by Licensed Practical Nurses and paraprofessionals.
  • Manage, plan, coordinate, and execute the patient's safe discharge from service.
  • Ensure physician's orders, discharge summaries, and communication to team members, patients, and family members adhere to company and regulatory requirements.
  • Communicate and consult with patient's physician as needed; manage and coordinate all aspects of assigned caseload.
  • Assist patients with activities of daily living.
  • Document content and process of all services provided in the internal system properly.
  • Transport and utilize medical equipment to and from clients'/patients' homes/facilities.
  • Travel to patients'/clients' homes and/or other facilities.
  • Perform other related duties and participate in special projects as assigned.

Requirements

  • CT LPN licensure.
  • BLS, ACLS, or CPR certification.
  • Minimum of 6 months to 1 year of demonstrated experience as an LPN.
  • Ability to put patients at ease and explain physician's instructions.
  • Excellent organization skills and ability to multi-task.
  • Experience working directly with people from diverse racial, ethnic, and socioeconomic backgrounds.
  • Knowledge of medical terminology.
  • Team player with the ability to work independently.
  • Valid driver's license or Non-Driver photo ID card (drivers preferred).

Schedule

  • Flexible hours with shifts from 6, 8, to 12 hours.
  • Day and night shifts available.
  • Part-time, per diem, or around your current work schedule.
  • Shifts assigned to provide 1:1 care in the home environment.
