Licensed Practical Nurse ( LPN ) – Day Shift - 12 Hour Shift

About the role

Park Manor Health & Rehabilitation is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our skilled nursing and rehabilitation team on the 7:00 AM–7:00 PM day shift. You'll provide exceptional nursing care to residents in our long-term care and rehabilitation community while working alongside an experienced interdisciplinary team dedicated to delivering outstanding, resident-centered care.

Responsibilities

• Provide direct nursing care to residents in a skilled nursing, rehabilitation, and long-term care setting.
• Assess, monitor, and document changes in resident conditions.
• Administer medications, treatments, and nursing interventions according to physician orders.
• Monitor resident response to medications and treatments.
• Collaborate with Registered Nurses (RNs), physicians, therapists, and other healthcare professionals to develop and implement individualized care plans.
• Accurately complete electronic nursing documentation.
• Respond promptly to changes in resident condition and communicate concerns appropriately.
• Supervise and support Certified Nursing Assistants (CNAs) to ensure quality resident care.
• Ensure compliance with state and federal regulations and facility policies.

Requirements

• Current Licensed Practical Nurse (LPN) license in good standing.
• Current CPR/BLS certification.
• Experience in skilled nursing, rehabilitation, long-term care, post-acute care, or geriatric nursing is preferred but not required.
• Strong clinical assessment, communication, and organizational skills.
• Ability to work collaboratively within an interdisciplinary healthcare team.

Pay

Base pay: $25.00–$29.55 per hour (based on experience).

Schedule

7:00 AM–7:00 PM day shift.
