Licensed Practical Nurse, LPN - Community-Based
Requirements
- Licensed Practical Nurse (LPN)
- Active LPN license issued by the Virginia Board of Nursing
- 2 years of experience providing mental health services to adults with serious mental illness
- At least 1 year of experience in medication management
- Strong knowledge of recovery principles and recovery-driven care
- Valid Virginia driver’s license
Responsibilities
- Provide recovery-based, person-centered clinical services through individual and group therapy, supporting mental health, daily living skills, and social development.
- Develop, implement, and regularly update treatment plans in collaboration with individuals, families, and the interdisciplinary team.
- Conduct comprehensive assessments and provide ongoing evaluation of symptoms, treatment responses, and overall progress.
- Support overall health needs by providing medication education, monitoring, administration, and addressing physical health concerns.
- Act as a liaison and advocate by coordinating with families and community agencies, offering education, crisis support, and connections to resources.

Pay
- $30.30 hour + mileage reimbursement
Schedule
- Monday–Friday, 8:00 AM–4:30 PM, plus rotating on-call coverage on consecutive days and weekend community coverage.
