Job Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our skilled nursing facility.

Responsibilities

  • Assists with new admissions.
  • Provide medical intervention as needed for emergent needs.
  • Communicate needs of residents to outside providers including but not limited to: Primary Care Provider, Home Health Agency, Pharmacy, etc.
  • Provide direct nursing care to residents, including administering medications, treatments, and monitoring vital signs.
  • Perform wound care, dressing changes, and other therapeutic nursing interventions as directed.
  • Observe residents for changes in health status and report any concerns.
  • Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, and feeding.
  • Collaborate with interdisciplinary teams to ensure quality resident care.
  • Maintain accurate and timely documentation of resident care, treatments, and progress in electronic medical records (EMRs) or paper charts.
  • Ensure compliance with all federal, state, and local regulations as well as facility policies and procedures.
  • Provide emotional support to residents and their families, fostering a compassionate and empathetic environment.
  • Participate in staff meetings, in-service training, and continuing education programs to stay updated on best practices.
  • Support infection control and safety protocols throughout the facility.
  • Supervise certified nursing assistants (CNAs) and provide direction as needed.
  • Other duties as assigned.

Requirements

  • Current Licensed Practical Nurse (LPN) license in the state of practice.
  • Experience in a skilled nursing facility or similar healthcare setting (preferred).
  • Basic Life Support (BLS) certification required.
  • Strong understanding of geriatric care and the needs of elderly residents.
  • Proficiency in using electronic health records (EHR) systems.
  • Excellent communication and interpersonal skills.
  • Ability to work in a fast-paced environment and handle multiple tasks with attention to detail.
  • Compassionate, patient-focused, and empathetic approach to care.
