Licensed Practical Nurse (LPN)

About the Role

A pediatric home health nursing role focused on providing one-on-one care in the patient's home, supporting pediatric patients with skilled nursing needs. Collaborates closely with RNs and clinical leadership to ensure quality care.

Responsibilities

• Deliver personalized nursing care to pediatric patients in their home
• Follow care plans with RN oversight
• Maintain patient relationships and ensure safety and comfort
• Adhere to CDC guidelines and utilize PPE for infection control
• Provide skilled nursing care, including G-tube, trach, or vent care as needed
• Use an easy-to-use portal for scheduling and communication

Requirements

• Valid Pennsylvania LPN or Multistate License
• Current TB test
• BLS CPR certification (in-person training required)
• Valid driver’s license
• Experience or willingness to train in G-tube, trach, or vent care
