Position Summary

We are seeking a dedicated and skilled Licensed Practical Nurse (LPN) to join our Medical Unit 4E team. The ideal candidate will demonstrate exceptional patient care skills, proficiency in telemetry, and a strong understanding of Epic systems.

Key Responsibilities

  • Distribute medications and assist with care.
  • Collaborate with a multidisciplinary team to ensure optimal patient outcomes.
  • Audit and evaluate patients' physical, psychosocial, environmental, and educational needs, working with the team to achieve desired outcomes.
  • Develop and implement care plans, including discharge planning, using critical thinking and clinical expertise.
  • Facilitate patient and family education tailored to individual needs.
  • Adapt to changes in the healthcare environment by embracing new technology and processes.
  • Communicate effectively with RNs and multidisciplinary caregivers regarding changes in patient conditions.
  • Attend departmental meetings and educational opportunities with a required participation of 80%, including in-person and webinar attendance.
  • Maintain confidentiality, adhere to HIPAA regulations and safety protocols, and document patient information accurately in the electronic and/or paper medical record systems.
  • Delegate and receive tasks appropriately, based on patient needs and scope of practice.
  • Participate in process improvement teams and support initiatives enhancing patient satisfaction and clinical outcomes.
  • Take responsibility for personal and professional development, setting yearly performance goals and advancing clinical expertise.
  • Serve as a steward of hospital resources, meeting unit productivity expectations and supporting staffing needs across departments when required.

Requirements

  • Active and valid BLS and LPN license.
  • Minimum of 1 year of relevant clinical experience.
  • Proficiency in Epic and telemetry, with a commitment to providing competent and compassionate patient care.
