Licensed Practical Nurse (LPN)

About the role

Human Touch Home Health is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to serve in the Allentown and surrounding Lehigh Valley communities, including Whitehall, Bethlehem, Emmaus, Macungie, Catasauqua, Copply, Fountain Hill, and nearby areas.

Responsibilities

• Provide skilled nursing visits in patients' homes
• Perform focused nursing assessments and report changes in patient condition
• Administer medications and treatments as ordered and within the LPN scope of practice
• Provide wound care and monitor healing
• Reinforce disease-management and medication education
• Maintain vital signs and patient response to treatment
• Communicate changes and concerns promptly to the supervising RN and care team
• Collaborate with nurses, therapists, physicians, and other members of the interdisciplinary team
• Complete timely and accurate clinical documentation using WellSky

Requirements

• Active Pennsylvania Licensed Practical Nurse (LPN) License
• Current CPR/BLS Certification
• Valid Driver's License and Reliable Transportation
• Strong Nursing, Communication, and Documentation Skills
• Ability to work independently in patients' homes
• Home Health Experience Preferred, but Not Required
• WellSky Experience is a Plus
