Licensed Practical Nurse (LPN)

About the Role

Seeking a Licensed Practical Nurse (LPN) for pediatric home health, providing focused, one-on-one care in patients' homes. The role involves supporting pediatric patients with skilled nursing needs, collaborating closely with registered nurses and clinical leadership. The LPN delivers personalized care, builds meaningful patient relationships, and follows clear care plans.

Responsibilities

• Administer skilled nursing care in a home setting
• Work under RN supervision and follow established care plans
• Maintain safety and hygiene standards per CDC guidelines
• Utilize the Employee Portal for scheduling and communication

Requirements

• Valid New Jersey LPN or Multistate License
• Physical exam within three years, PPD or Chest X-Ray, and BLS CPR card (in person)
• Valid driver's license
• Experience with G-tube, trach, vent preferred or willingness to train
