About the role
EnCircle helps people with developmental disabilities lead person-centered and abundant lives. We are seeking a full-time LPN Health Coordinator to oversee the medical component of group home operations.
Responsibilities
- Provide skilled nursing care in accordance with physician orders, nursing standards, and agency policies.
- Maintain communication with service coordinators or other assigned contacts and day support managers.
- Advocate for individuals' healthcare needs and access to services.
- Coordinate medical appointments and follow-up care as needed.
- Complete nursing assessments, notes, health summaries, and incident reports follow-up documentation.
- Participate in nurse on-call rotation as directed by Nursing Supervisor.
- Perform other related duties as required and assigned.
Requirements
- Must be 18 years of age or older.
- Current, Valid LPN license.
- BLS, CPR.
- Valid driver’s license and proof of insurance required.
- Satisfactory Driving Record with no negative points balance.
- Ability to pass a background check and drug screen.
- Must have responsible work ethic and good communication skills.
- Must be compassionate and caring.
- Availability for home visits in community.

Pay
Competitive salary starting at $23.50 per hour (commensurate with experience).
Schedule
Monday-Friday, Daytime hours. Rotational on-call.
Driving Specific
- Must not have any of the following violations within the last 5 years:
- Reckless driving, including speeding 20mph or more over the posted speed limit.
- Driving under the influence/impaired/intoxicated or refusing a blood/breath test.
- Manslaughter or negligent homicide.
- Driving after being declared a habitual offender.
- Driving on a suspended or revoked license.
- Driving a commercial motor vehicle while disqualified, under the influence, with a blood alcohol level of .04 or higher, refusing a blood/breath test, or violating an out-of-service order.
- Injuring a person while racing - felony.
- Failure to stop at the scene of a crash where there is injury, death, or property damage of $1000 or more.
- Vehicular assault or willful stopping, impeding, or damaging a vehicle.
- Blocking access to the service facility.
- Attempting to elude police.
- Operating an unsafe vehicle.
- Vulnerable Road User – Death.
