About the role

EnCircle helps people with developmental disabilities lead person-centered and abundant lives. We are seeking a full-time LPN Health Coordinator to oversee the medical component of group home operations.

Responsibilities

  • Provide skilled nursing care in accordance with physician orders, nursing standards, and agency policies.
  • Maintain communication with service coordinators or other assigned contacts and day support managers.
  • Advocate for individuals' healthcare needs and access to services.
  • Coordinate medical appointments and follow-up care as needed.
  • Complete nursing assessments, notes, health summaries, and incident reports follow-up documentation.
  • Participate in nurse on-call rotation as directed by Nursing Supervisor.
  • Perform other related duties as required and assigned.

Requirements

  • Must be 18 years of age or older.
  • Current, Valid LPN license.
  • BLS, CPR.
  • Valid driver’s license and proof of insurance required.
  • Satisfactory Driving Record with no negative points balance.
  • Ability to pass a background check and drug screen.
  • Must have responsible work ethic and good communication skills.
  • Must be compassionate and caring.
  • Availability for home visits in community.

Pay

Competitive salary starting at $23.50 per hour (commensurate with experience).

Schedule

Monday-Friday, Daytime hours. Rotational on-call.

Driving Specific

  • Must not have any of the following violations within the last 5 years:
  • Reckless driving, including speeding 20mph or more over the posted speed limit.
  • Driving under the influence/impaired/intoxicated or refusing a blood/breath test.
  • Manslaughter or negligent homicide.
  • Driving after being declared a habitual offender.
  • Driving on a suspended or revoked license.
  • Driving a commercial motor vehicle while disqualified, under the influence, with a blood alcohol level of .04 or higher, refusing a blood/breath test, or violating an out-of-service order.
  • Injuring a person while racing - felony.
  • Failure to stop at the scene of a crash where there is injury, death, or property damage of $1000 or more.
  • Vehicular assault or willful stopping, impeding, or damaging a vehicle.
  • Blocking access to the service facility.
  • Attempting to elude police.
  • Operating an unsafe vehicle.
  • Vulnerable Road User – Death.
