About the role

Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states. We are seeking a Full-Time Licensed Practical Nurse (LPN) in Wrightsville, PA to deliver care to individuals with disabilities in a community setting, utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation.

Responsibilities

  • Provide treatment and health care procedures ordered by a physician and required by the standards of professional practice or law
  • Ensure individuals attend medical appointments per their Individual Support Plan (ISP)
  • Advise supervisor of all concerns and issues related to the provision of nursing services
  • Create and distribute medical/health reports detailing general health, health care goals, and physician recommendations
  • Develop and update Individual Support Plans (ISP)/Person Centered Plans (PCP) with an interdisciplinary team to include any concerns, observations, and behavioral developments
  • Train employees on the medical needs of assigned individuals
  • Maintain the standards of professional nursing by interacting with individuals, significant others, and health team members
  • Monitor health and medical conditions of individuals and refer for medical services as needed
  • Monitor the use of medications through medication administration logs
  • Avoid the use of medications through medication administration logs
  • Ensure health service supplies are available and replenished as needed
  • Ensure program documentation and billable records are completed accurately and timely
  • Build a support network for individuals by developing and maintaining relationships with community members, businesses, and families
  • Participate with individuals in community, social, and recreational activities
  • Participate in circles of support for personal futures planning
  • Follow strict OSHA procedures regarding blood borne pathogens

Requirements

  • Bachelor’s degree or equivalent experience in nursing field
  • Minimum of one year of experience providing nursing services to individuals with disabilities
  • Valid R.N. license
  • Valid driver’s license with a satisfactory driving record
  • Complete all state and agency required training per state guidelines
  • Ability to calculate figures related to medication dosages
  • Understanding and commitment to community-based support for persons with disabilities
  • Ability to exercise good judgment and remain calm in crisis situations
  • Ability to establish a comfortable and supportive relationship with an individual receiving supports
  • Excellent verbal and written communication skills
  • Ability to effectively present facts and recommendations
  • Ability to effectively communicate with program participants, third-party agencies, staff and members of the community
