Pay
- Full-time/Part-time LPN wage range: $36.00 to $48.50 per hour (based on newly announced wage grid)
- Per Diem rate: $42.00 plus differential
- Sign-on bonus: $2,500.00 (prorated for part-time)

Qualifications
- Current unencumbered license to practice nursing in the state where the center is located
- Proficiency in basic computer skills, including electronic health records (PCC is a plus)
Responsibilities
- Implement systems addressing each resident’s needs, with a broad scope of knowledge to care for medically complex and mentally compromised residents
- Assess healthcare needs of residents, make nursing diagnoses, and develop interdisciplinary care plans utilizing available resources
- Communicate care plans effectively to all care providers to ensure continuity of care
- Establish and maintain functional communication with staff, other disciplines, residents, families, medical staff, and the community
- Treat residents, staff, and facility community with respect and dignity
- Maintain honesty, confidentiality, and respect for residents and facility property and information
