About the Role
We are seeking a Licensed Practical Nurse (LPN) to provide high-quality care in a home health setting, supporting patients in Americare communities and in-home environments. This role focuses on patient-centered care, coordination with care teams, and maintaining a safe, comfortable, and home-like environment that supports each patient's physical, emotional, and social well-being.
What You'll Do
- Provide direct patient care in home health and community-based settings
- Serve as a liaison between Americare Home Health and community teams
- Assess patient needs and support individualized care plans
- Implement nursing standards, policies, and care protocols
- Monitor patient condition and report changes to the care team
- Collaborate with physicians, nurses, therapists, and leadership
- Provide guidance and support to caregivers and nursing staff
- Ensure accurate documentation and compliance with regulations
Requirements
- Graduate of an approved Practical Nursing (LPN) program
- Current, active LPN license in the applicable state
- Maintain required state certifications and continuing education
Qualifications
- Basic knowledge of home health or senior living care
- Strong attention to detail and clinical accuracy
- Ability to work independently and manage priorities
- Strong communication and teamwork skills
- Commitment to maintaining patient confidentiality and dignity
- Previous experience in home health, long-term care, or senior living preferred

Schedule & Pay
Wages: $30-$55 Per Visit + Mileage Reimbursement. Flexible Schedule available. Benefits may vary based on full-time, part-time, PRN, or visit-based status.
