About the role
A Licensed Practical Nurse (LPN) in a memory care and assisted living facility provides direct medical care, manages medications, and supports residents' cognitive and physical needs. Working under the supervision of a Registered Nurse (RN), LPNs also oversee frontline caregivers, assess resident health, and communicate with families and doctors. The ideal candidate will possess excellent clinical skills, strong leadership abilities, and a passion for working with seniors in a long-term care setting.
Responsibilities
- Medication Administration: Safely prepare, administer, and document medications and treatments. Conduct regular narcotic counts and ensure compliance with pharmacy and state regulations.
- Health Monitoring & Assessments: Assess residents' conditions upon move-in and complete ongoing, routine evaluations. Monitor vital signs and track changes in physical health or cognitive behaviors.
- Memory Care Focus: Tailor care to residents with Alzheimer's or dementia. Provide redirection, manage memory-related behavioral challenges, and offer structured cognitive support.
- Care Plan Execution: Follow Individualized Service Plans (ISPs) designed by the wellness director or RN, adjusting care as resident needs evolve.
- Staff Supervision: Supervise Certified Nursing Assistants (CNAs), Caregivers, Medication Aide/Tech, and care partners, offering guidance and assistance during daily routines.
- Family & Doctor Liaison: Communicate with physicians regarding orders and updates. Provide compassionate updates to families and respond to their concerns.
- Resident Assessments: Conduct thorough assessments of new and existing residents to identify healthcare needs, preferences, and goals. Continuously update care plans based on changes in health status.
- Training & Mentorship: Educate resident assistants and medication technicians on dementia-specific best practices, infection control, and emergency procedures.
- Daily Oversight: Ensure staff are properly following structured daily routines, which are critical for lowering stress and supporting memory care residents.
- Advocacy: Provide emotional and psychosocial support to residents and their families, connecting them to community resources.
- Problem-Solving & Critical Thinking: Handle dynamic and evolving healthcare situations by applying critical thinking skills and effectively responding to emergencies both in person and remotely, addressing changes in resident health, and staff challenges.
- Other duties assigned are appropriate to the position.
Requirements
- Must possess a nursing degree from an accredited college.
- Must possess a current, unencumbered, active license to practice as a Licensed Practical Nurse in this state.
- Nursing experience in assisted living, Alzheimer/dementia, long term care, geriatric rehab, sub-acute and/or critical care clinical setting.
- Thorough knowledge of CMS and State Regulations regarding patient care in an ALF/IL/MC facility.
- Experience in electronic medical records documentation.
- Strong leadership or mentorship and communication skills.
- Knowledge of State and Federal regulations and compliance requirements.
- Excellent organizational and time management skills.
- Must have and maintain an active CPR/BLS during employment.
- Maintain confidentiality of all resident care information.
- Compassion for and desire to work with the elderly.
