About the role
We are seeking compassionate, dedicated Licensed Practical Nurses (LPNs) to join our team. As an LPN, you will provide high-quality nursing care while working closely with physicians, Registered Nurses, and the interdisciplinary care team to promote the health, comfort, and well-being of our residents.
Responsibilities
- Administer medications and treatments as prescribed by physicians and monitor residents for therapeutic effectiveness and adverse reactions.
- Assess, monitor, and document resident conditions, reporting changes promptly to Registered Nurses and physicians.
- Obtain and accurately record vital signs and other clinical observations.
- Maintain complete, accurate, and timely clinical documentation in accordance with facility policies and regulatory requirements.
- Assist with admissions, transfers, and discharges as assigned.
- Communicate effectively with residents, families, physicians, and interdisciplinary team members.
- Provide compassionate, resident-centered nursing care while promoting dignity, independence, and quality of life.
- Supervise and collaborate with Certified Nursing Assistants to ensure exceptional resident care.
- Respond appropriately to emergencies and changes in resident condition.
Requirements
- Graduate of an accredited Licensed Practical Nursing (LPN) program.
- Current Licensed Practical Nurse (LPN) license in good standing with the Commonwealth of Pennsylvania.
- Previous long-term care, rehabilitation, or skilled nursing experience preferred, but not required.
- Current CPR certification preferred.
- Strong clinical, communication, organizational, and teamwork skills.
