Responsibilities
- Provide care to residents in accordance with their individualized care plans.
- Assist with daily living activities such as bathing, dressing, and eating.
- Monitor and record vital signs and other health data.
- Administer medications as prescribed by physicians.
- Participate in shift handovers and report resident status to supervisory staff.
- Collaborate with interdisciplinary teams to develop and implement care plans.
Qualifications
- NY RN / LPN License
- Must have 3 years experience as an LPN Licensed Practical Nurse in LTC
- Able to adapt and take on all responsibilities of the LPN role
