About the role
The primary purpose of the lead LPN/LVN is to provide direct nursing care to the residents.
Responsibilities
- Ensure the facility meets the minimum staffing levels for nurses and nurse aides; complete required posting of nursing staff data as required.
- Audit nursing documentation in the clinical record for appropriate and relevant entries.
- Coordinate Pre-certification and recertification in accordance with facility policies for Medicare Advantage and commercial insurance payers.
- Provide licensed nursing care to residents that is consistent with the written plans of care.
- Communicate with primary care physicians and members of the comprehensive care planning team regarding the status of residents; follow up on critical labs, order requests and physician notifications.
- Collaborate with the Staff Development Coordinator for team member training, skill development and competencies in accordance with facility policies.
- Maintain list(s) of equipment repairs or needs required for resident care.
Requirements

- Must possess the ability to make independent decisions when circumstances warrant such action.
- Must possess the ability to interpret the department’s policies and procedures to personnel, residents, visitors and government agencies as required.
- Must be able to assess residents for conditions which may be aided by assistive or adaptive devices; recommend/request in accordance with facility policies.
- Must be able to communicate effectively with residents, families, and other healthcare professionals.
- Must be able to perform tasks independently and manage time effectively.
- Must be able to maintain confidentiality and adhere to HIPAA regulations.
Qualifications
- Current license to practice as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in the state of Georgia.
- At least one year of experience in a healthcare setting preferred.
- Bachelor's degree in Nursing or related field preferred.
Skills
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Knowledge of infection prevention and control procedures.
- Proficiency in computer systems used for patient care documentation.
