Responsibilities
- Provides nursing services to individuals and families in their residence under the supervision of a registered nurse.
- Performs prescribed individualized patient care to patients as delegated.
- Participates in patient case conferences as appropriate; is an active member of the interdisciplinary team.
- Affords assistance to the RN in the preparation, evaluation, and modification of the patient's plan of care as appropriate.
- Observes, documents, and reports to the RN as appropriate, the patient's physical and emotional needs and reactions to care.
Requirements
- Must be a Licensed Practical Nurse in Vermont and New Hampshire or hold a compact license.
- Graduate of an accredited school as a Licensed Practical Nurse.
- One year of experience as an LPN in a home health care setting is preferred.
- Unencumbered LPN license in VT and NH or compact.
Qualifications
- Driver’s license, minimum requirements on car insurance, and a clean driving record.
Skills
- Broad independence in providing nursing services.
- Active participation in interdisciplinary teams.
- Ability to observe, document, and report patient needs and reactions.
