Responsibilities

  • Provides nursing services to individuals and families in their residence under the supervision of a registered nurse.
  • Performs prescribed individualized patient care to patients as delegated.
  • Participates in patient case conferences as appropriate; is an active member of the interdisciplinary team.
  • Affords assistance to the RN in the preparation, evaluation, and modification of the patient's plan of care as appropriate.
  • Observes, documents, and reports to the RN as appropriate, the patient's physical and emotional needs and reactions to care.

Requirements

  • Must be a Licensed Practical Nurse in Vermont and New Hampshire or hold a compact license.
  • Graduate of an accredited school as a Licensed Practical Nurse.
  • One year of experience as an LPN in a home health care setting is preferred.
  • Unencumbered LPN license in VT and NH or compact.

Qualifications

  • Driver’s license, minimum requirements on car insurance, and a clean driving record.

Skills

  • Broad independence in providing nursing services.
  • Active participation in interdisciplinary teams.
  • Ability to observe, document, and report patient needs and reactions.
