Licensed Practical Nurse for Personal Care Management Program

About the role

You will work in the Personal Care Attendant (PCA) Evaluation Department, making a significant impact on the lives of individuals with disabilities through assessments for the MassHealth PCA program.

Responsibilities

• Perform annual reevaluations of MassHealth PCA program applicants in accordance with 130 CMR 422.422(C)
• Submit evaluations within the specified timeframe
• Respond to hours adjustment requests promptly
• Communicate with MassHealth and offer professional input regarding PCA evaluations
• Adhere to PCA Program standards and schedule reevaluation visits
• Document progress notes in the database in a timely manner
• Communicate with PCA Eval Department Assistants about no-shows, no contacts, hospitalizations, and termination requests
• Submit monthly mileage sheets within the required timeframe
• Work collaboratively with other members of the PCA Program
• Attend monthly staff meetings and report suspicions of fraud and abuse to relevant agencies
• Exercise sound judgment and maintain confidentiality
• Represent the agency professionally and provide culturally sensitive services
• Perform other duties as assigned by the Manager

Requirements

• Current Licensed Practical Nurse (LPN) license in Massachusetts
• At least 2 years recent experience in direct care of the elderly or disabled population
• Associate’s degree preferred, but not required
• Familiarity with MassHealth PCA Program preferred
