Licensed Practical Nurse - Fayetteville Family Practice

About the Role
Fayetteville Primary Care is a busy, fast-paced outpatient office that offers dynamic learning opportunities focused on patient-centered medical home and continual advancements. We use a team-based nursing model to meet the needs of a diverse patient population from newborn to geriatric. Training includes a structured orientation period to become familiar with practice workflows.
Schedule: Monday through Friday day shifts (7:30 AM – 5:00 PM) with occasional evenings until 7:30 PM and occasional Saturday mornings (8:00 AM – 12:00 PM). The office is closed on eight major holidays or observed holidays.
Responsibilities
- Perform therapeutic nursing interventions, including obtaining medical history and assessing present illness.
- Care for patients across the lifespan (newborn to geriatric).
- Administer medications and vaccines after completing on-site training.
- Assist with procedures and perform point-of-care testing.
- Document care in the EPIC electronic health record system.
- Collaborate with an interdisciplinary team of registered nurses, medical assistants, and providers.
- Ensure quality nursing care in accordance with the New York Nurse Practice Act, National Standards of Practice, and Nursing and Clinical Service standards.
- Utilize the Nursing Process in the provision of direct care to patients and families.
- Follow Preventive Medicine Guidelines.
Requirements
- Graduation from an accredited school for Licensed Practical Nursing.
- Current New York State LPN licensure.
- Maintain current BLS/CPR certification.
- Participate in orientation, continuing education, and maintain knowledge and skills related to specific areas of expertise.
Skills
- Strong rapport with interdisciplinary care teams.
- Ability to lift 25 pounds frequently and occasionally lift up to 50 pounds.
- Ability to stand, walk, sit, bend, twist, squat, and reach.

Pay
Pay range: $26.50 – $35.10 per hour, based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
