About the role
Fayetteville Primary Care is a busy, fast-paced outpatient office that offers dynamic learning opportunities focused on patient-centered medical home and continual advancements. We use a nursing model with a team approach to meet the needs of a diverse patient population from newborn to geriatric. Training includes a structured orientation period to become familiar with practice workflows.
Schedule
- Eight-hour day shifts, Monday through Friday.
- Occasional evenings until 7:30 PM.
- Occasional Saturday mornings, 8:00 AM – 12:00 PM.
- Office closed on eight major holidays (or observed holidays).
Responsibilities
- Perform therapeutic nursing interventions, including obtaining medical history and assessing present illness.
- Care for patients across the lifespan (newborn to geriatric).
- Administer medications and vaccines after completing on-site training.
- Assist with procedures and perform point-of-care testing.
- Document care using the EPIC electronic health record system.
- Collaborate within an interdisciplinary team of RNs, medical assistants, and providers.
- Ensure quality nursing care in accordance with the New York Nurse Practice Act, National Standards of Practice, and Nursing and Clinical Service standards.
- Utilize the Nursing Process in the provision of direct care to patients and families.
- Follow Preventive Medicine Guidelines.
Requirements
- Graduation from an accredited school for Licensed Practical Nursing.
- Current New York State LPN licensure.
- Maintain current BLS/CPR certification.
- Participate in orientation, continuing education, and maintain knowledge and skills related to specific areas of expertise.

Pay
$26.50 – $35.10 per hour, based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may vary by location.
Physical demands
- Medium work: must be able to lift 25 pounds frequently and up to 50 pounds occasionally.
- Ability to stand, walk, sit, bend, twist, squat, and reach.
