Licensed Practical Nurse Case Manager

Position Summary

The Case Manager at Freedom Behavioral Lake Charles plays a vital role in supporting patients throughout their treatment journey by coordinating care and ensuring each patient receives the services, resources, and support needed for successful recovery and continuity of care.

Responsibilities

• Conduct assessments
• Afford individualized treatment and discharge planning
• Collaborate closely with physicians, nurses, therapists, families, and community partners to address each patient’s mental health and social needs
• Cook up follow-up services
• Connect patients with community resources
• Support safe and effective discharge planning
• Serve as an advocate and key point of communication between patients, families, and the treatment team while helping promote positive patient outcomes and stability

Requirements

• Education: A psychology major is preferred or nursing education
• Licenses: Nursing License, Social Worker License, &/or LPN
• Experience: 1+ years coordinating utilization review and discharge planning activities
