About the role
The Licensed Practical Nurse (LPN) is responsible for preparing patients for examinations and delivering individualized patient care by promoting and restoring health through the nursing process. The LPN collaborates with physicians and multidisciplinary care team members to provide physical and emotional support to patients in a Primary Care setting. The LPN will also contribute to the clinic’s efforts to achieve and maintain Patient-Centered Primary Care Home (PCPCH) recognition and support the effective use and optimization of electronic health records (EHR) and other health information technology systems.
Responsibilities
- Escorts patients to exam rooms, prepares them for examination and treatment, obtains vital signs, and enters medical history into the electronic medical record (EMR).
- Prepares exam rooms and instruments for examinations and testing and maintains appropriate medical supply inventory.
- Provides direction and support to Medical Assistants (MAs) regarding patient care activities.
- Conducts patient assessments and prioritizes care based on the patient’s condition and needs within the LPN Scope of Practice.
- Assists in developing and implementing individualized care plans in collaboration with providers and healthcare team members.
- Performs treatments and therapeutic nursing interventions as ordered by the provider in an accurate and timely manner.
- Provides age-appropriate, culturally competent patient care and education tailored to individual patient and family needs.
- Documents patient assessments, interventions, responses to treatment, and progress toward care goals accurately and timely in the medical record.
- Verifies patient identity prior to providing or documenting care and supports efficient patient flow and care coordination.
- Collects laboratory specimens in accordance with organizational guidelines.
- Adheres to universal safety precautions, including safe medication administration and proper disposal of medical waste.
- Initiates emergency resuscitative measures in accordance with established adult and pediatric life support protocols.
- Maintains patient confidentiality and compliance with HIPAA requirements.
- Provides care in a nonjudgmental and nondiscriminatory manner that respects patient autonomy, dignity, rights, and diversity.
- Maintains professional competency through continuing education and professional development activities.
- Assists with desktop triage and other clinical support functions as assigned.
- Complies with organizational policies, procedures, Code of Conduct, Compliance Plan, and all applicable federal, state, and local regulations.
- Performs other duties as assigned to support departmental and organizational operations.

Requirements
- Current Licensed Practical Nurse (LPN) license issued by the State of Oregon.
- Current CPR certification.
- Demonstrated ability to perform nursing skills within the LPN Scope of Practice.
- Current valid driver’s license, reliable transportation, and proof of automobile insurance.
- All vaccinations up to date, including but not limited to Hepatitis, MMR, and Tetanus. COVID-19 vaccination preferred or approved medical/religious exemption on file.
- Ability and willingness to work three (3) 12-hour shifts, including some weekends.
- Proficient computer skills, including experience with electronic health records (EHR), Microsoft Office Suite, typing, and 10-key preferred.
- No suspension, exclusion, or debarment from participation in federal healthcare programs (e.g., Medicare/Medicaid).

- Strong verbal and written communication skills to effectively collaborate with healthcare team members, patients, and families.
- Experience working with diverse populations and varying communication styles.
- Ability to handle and resolve difficult situations professionally and effectively.
- Bilingual or translation capabilities preferred.
- Familiarity with quality improvement and process improvement methodologies.
- Working knowledge of federal and state regulations impacting clinic operations.
- Ability to use Health Information Technology accurately and efficiently.
- Ability to build positive working relationships while communicating in a tactful, respectful, and direct manner.
- Strong organizational skills and attention to detail.
Physical Demands
- Typical office environment requires standing, sitting, walking, bending, and lifting to 25 plus pounds.
- Ability to travel and drive to in-person meetings or be on-site as required.
- May be exposed to patient and environment conditions such as unpleasant sights, smells, and contagious diseases.
