Licensed Practical Nurse

Type: Full Time • Location: Orange County (primarily serving an Assisted Living Facility in 32810 for Home Health Insulin Visits)

Responsibilities

• Provide high-quality, compassionate care to home health patients according to the patient’s plan of care
• Use assessment tools to manage medical needs and coordinate with other disciplines
• Serve as a patient advocate, communicating with physicians, clinical managers, and RNs
• Make a meaningful impact on patients' lives every day

Requirements

• Graduate of an accredited school of nursing
• Active LPN license in Florida
• Valid Florida driver’s license, active vehicle insurance, and clear MVR
• Minimum 1 year of nursing experience required; Home Health experience is a plus
• IV certification preferred
• Strong initiative and independent judgment
• Alignment with the agency’s mission and values
