About the role

Traditional Home Health Care (Pediatric & Adult Skilled) provides exceptional skilled nursing and aide services to pediatric and adult patients in their homes. We are seeking a compassionate Licensed Practical Nurse (LPN) to deliver meaningful one‑on‑one care, with flexible scheduling and the opportunity to make a real difference in patients’ lives. Service location: Hazleton, PA and surrounding areas.

Responsibilities

• One‑to‑one patient care in home/school environment
• Implement and document the plan of care to ensure quality and continuity of care for patient
• Administration of prescribed medication, treatments and therapies
• Patient assessments; coordination of care
• Report any changes in the client condition to Clinical Manager and physician
• Health promotion, teaching, and training of family members

Pay

Rate: $27‑$32 per hour

Schedule

• Weekday and weekend shifts, full‑time and part‑time schedules available
• Morning, evening or overnight shifts
• 8‑hour, 10‑hour, or 12‑hour shifts (full shift work)
• PRN, Part‑Time, or Full‑Time options
• Immediate need in Hazleton, PA: Friday and Saturday 10 pm – 7 am
• Locations served: Luzerne County
