Traditional Home Health Care Pediatric Division is seeking a compassionate Licensed Practical Nurse (LPN) to join our team. Responsibilities

• One to one patient care in home/school environment
• Implement and document the plan of care to ensure quality and continuity of care for patient
• Administer and document the administration of prescribed medication, treatments, and therapies
• Perform patient assessments; coordinate care
• Report any changes in the client condition to Clinical Manager and physician
• Engage in health promotion, teaching, and training of family members

Requirements

• Current Pennsylvania LPN license
• Experience in pediatric or adult home health care preferred
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Valid driver’s license and reliable transportation

Qualifications

• High school diploma or equivalent
• Completion of an accredited LPN program
• Passing scores on the NCLEX-RN

Skills
