Responsibilities

  • Assess patient status and follow the plan of care in collaboration with physicians, nursing supervisors, patients, and family members.
  • Administer patient care, manage and evaluate the care plan, and teach patients in accordance with physician orders.
  • Report patient status changes, care, and progress to physicians and nursing supervisors continuously.
  • Revise the plan of care in consultation with the physician.
  • Prepare, maintain, and submit meticulous documentation, including interactions, case conferences, medication changes, time points, and plan-of-care updates.
  • Act as a liaison and maintain contact with patients, therapists, nursing supervisors, and other healthcare team members.
  • Inform patients of available services and make referrals to community resources as needed.

Requirements

  • Current license as a Practical Nurse (LPN) in Pennsylvania.
  • Current driver’s license and ability to travel to case assignments.
  • Excellent assessment, documentation, and time-management skills.
  • Ability to work independently with minimal supervision.
  • Strong interpersonal, verbal, written, and organizational skills.
  • Acute care and/or home health experience preferred.
