Licensed Practical Nurse
Position Overview
Maintain health conditions of individuals served and ensure quality nursing services are provided.
Administer medications, maintain accurate documentation, and oversee staff responsible for medication administration.
Provide prescribed medical treatments according to physician orders and deliver personal care services as needed.
Conduct health assessments and implement healthcare services within each Individual Service Plan (ISP)
Monitor and respond to changes in individuals' health conditions
Provide staff training on infection control, universal precautions, medication administration, and other healthcare topics
Serve as a liaison with physicians, healthcare providers, and community agencies to coordinate quality care
Educate individuals served on topics including self-medication, prescribed treatments, smoking cessation, and health promotion.
Requirements
- Current Licensed Practical Nurse (LPN) license in the state of residence
- Associate's Degree in Nursing or related field, or state nursing certificate
- Minimum one (1) year of nursing experience working with the population served preferred
- Valid driver's license, current vehicle registration, and proof of auto insurance
- Strong communication and interpersonal skills with the ability to work effectively with individuals served, families, and interdisciplinary teams
