Licensed Practical Nurse

About the Role

Provide nursing service, documentation, and record review. Oversee medical information, verify follow-up and documentation, perform chart reviews, and manage appointments.

Responsibilities

• Ensure documentation is accurate with follow-up noted.
• Complete and ensure completion of routine work, including but not limited to: doctors' orders, follow-up appointments/tests/specialists, maintain tickler, face sheets, BM charts, Braden scales, self-medication checklist, and schedule appointments as needed.
• Provide bedside care.
• Administer medication and document therapeutic effects.
• Take vital signs (temperature, blood pressure, pulse, and respiration).
• Assist with resident comfort.
• Collect samples.
• Provide wound care.
• Help coach staff as necessary.
• Make and follow up on doctor's orders, appointments, etc.

Requirements

• High School diploma.
• 2+ years of experience.
• Up-to-date first aid and CPR certification.
• Current LPN license in the state.
• Exceptional staff training and customer service skills.
• Compassionate and friendly demeanor.

Physical Requirements

• Regularly required to communicate clearly and effectively with staff and consumers.
• Frequently required to travel.
• Occasionally required to lift and/or move up to 25 pounds.
