Licensed Practical Nurse
RiseWell Community Services is seeking a dedicated Licensed Practical Nurse (LPN) for our Residential Program in Brentwood, Sunken Meadow, NY. This position offers a unique opportunity to make a meaningful impact in the lives of individuals living with serious mental illness by providing community-based, recovery-oriented support.
About the role
As a member of a multidisciplinary residential team, you will provide health services including medication management to people with psychiatric disabilities in a transitional licensed residential program.
Responsibilities
- Provide health services including medication management
- Support individuals with psychiatric disabilities in a residential setting
- Collaborate with a multidisciplinary team to deliver recovery-oriented care
Requirements
- Currently licensed as a Licensed Practical Nurse by the New York State Education Department
- Valid and clean NY State driver license
- Minimum of 1 year of psychiatric experience in a treatment or rehabilitation setting
- Fingerprinting, criminal record check, and approval from NYS Office of Mental Health
