Pillar Care Continuum is dedicated to enhancing the lives of people with disabilities and other special needs by supporting personal growth, independence, and participation in the community. Founded in 1953 to meet the needs of children with cerebral palsy and their families, Pillar Care has steadily grown and now delivers life-affirming services to infants, children, adults, and their families throughout northern New Jersey through 27 program locations. Pillar Care provides specialized programs to meet a wide range of educational, therapeutic, and social needs of people with various disabilities. The agency serves over 1,650 families each year and employs 700 full- and part-time employees.
Responsibilities
- Participates in initial evaluations by securing and reviewing all pertinent medical records and assessing the physical well-being of consumers.
- May participate in the development of an Individual Habilitation Plan by including health status and health care needs; participate in yearly re-evaluations.
- Assesses the daily health status of consumers, treats medical problems as appropriate, and notifies the Service Coordinator of significant observations and concerns.
- Administers prescribed medication to consumers and first aid to consumers and/or staff.
- Maintains medical records including regular anecdotal notes indicating significant developments and/or observations; writes reports as necessary.
- Organizes medical and dental screening clinics; participates in clinics as required by health care providers.
- Participates in the development of new or revised health, safety, and emergency procedures; conducts and participates in in-service health education for consumers and staff; may provide health information and/or counseling to families.
- Administration of tube feedings and care of G-Tube site.
- Performs other related tasks as assigned by supervisor or program director.
Requirements
- LPN License required; graduate of an accredited nursing program licensed in the State of New Jersey
- Minimum of 1 year experience in an Adult Program, Hospital, Nursing Home, Clinic, or Group Home setting or an equivalent combination of education and experience
- Strong knowledge of health regulations, first aid, and emergency procedures
- Excellent communication and interpersonal skills, with the ability to work effectively with consumers, guardians, and staff
- Able to manage multiple tasks and remain calm in emergencies
- Valid driver's license
- Fingerprint examination and approval
- Successful TB screening
Qualifications
- Must successfully complete a criminal background check and receive clearance from the Department of Human Services, confirming they are not disqualified, before beginning work with individuals served by the organization.
- Must consent to having their name checked against the Central Registry of Offenders Against Individuals with Developmental Disabilities.
- Required to complete an application for a name check against the Child Abuse Registry Information (CARI).
- Required to complete an application to have their name checked against the Child Abuse Registry Information (CARI).
